Hextan Losartan K. Hextan Plus Losartan K 50 mg, hydrochlorothiazide 12.5 mg
Indications/Uses
Hextan HTN, alone or in combination w/ other antihypertensives. Hextan Plus Essential HTN in patients whose BP is not adequately controlled on losartan or hydrochlorothiazide alone.
Dosage/Direction for Use
Hextan Initially, 50 mg once daily. Patient w/ intravascular vol-depletion & history of hepatic impairment 25 mg daily. Hextan Plus 1 tab (losartan K 50 mg/hydrochlorothiazide 12.5 mg) once daily, may be increased to 1 tab (losartan K 100 mg/hydrochlorothiazide 25 mg) once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Hextan Plus Therapy resistant hypokalaemia or hypercalcaemia. Severe hepatic impairment; cholestasis & biliary obstructive disorders. Refractory hyponatraemia. Symptomatic hyperuricaemia/gout. Severe renal impairment (ie, CrCl <30 ml/min). Anuria. Patients w/ DM or renal impairment (GFR <60 ml/min/1.73 m2). 2nd & 3rd trimester of pregnancy.
Special Precautions
Symptomatic hypotension. Hepatic & renal impairment. Discontinue when pregnancy is detected. Pregnancy & lactation. Hextan Plus Electrolyte imbalance. Losartan: Closely monitor patients w/ angioedema. Hypotension & intravascular vol depletion; electrolyte imbalances; renal transplantation; primary aldosteronism; heart failure; ethnic differences. Hydrochlorothiazide: Hypotension. Metabolic & endocrine effects. W/ or w/o history of allergy or bronchial asthma. Exacerbation or activation of SLE. Galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption.
Adverse Reactions
Hextan Facial edema, fever, orthostatic effects, syncope. Angina pectoris, 2nd degree AV block, CVA, hypotension, MI, arrhythmias; anorexia, constipation, dental pain, dry mouth, flatulence, gastritis, vomiting; anemia; gout; arm, hip, knee & musculoskeletal pain, joint swelling, stiffness, arthralgia, arthritis, fibromyalgia, muscle weakness; anxiety, confusion, depression, dream abnormality, hypesthesia, decreased libido, memory & sleep disorder, somnolence, tremor, vertigo; dyspnea, bronchitis, pharyngeal discomfort, epistaxis, rhinitis, resp congestion; alopecia, dermatitis, dry skin, ecchymosis, erythema, flushing, photosensitivity, pruritus, rash, sweating, urticaria; blurred vision, burning/stinging in the eye, conjunctivitis, taste perversion, tinnitus, decreased in visual acuity; impotence, nocturia, urinary frequency & UTI; increased BUN; decreased Hb & hematocrit; elevation of liver enzymes. Hextan Plus Losartan: Insomnia; headache, dizziness, cough, upper resp infection, nasal congestion; sinusitis, sinus disorder; abdominal pain, nausea, dyspepsia; muscle cramps, back/leg pain, myalgia; asthenia, fatigue, chest pain; hyperkalemia, mild reduction of hematocrit & Hb. Hydrochlorothiazide: Cephalalgia.
Drug Interactions
Decreased AUC w/ phenobarb. Hextan Plus Losartan: Rifampicin & fluconazole. Increase in serum K w/ spironolactone, triamterene, amiloride; reduced lithium excretion. Attenuation of antihypersensitivity effect w/ NSAIDs (ie, selective COX-2 inhibitors, ASA) & non-selective NSAIDs. Increased risk of hypotension w/ TCA, antipsychotics, baclofen, amifostine. Hydrochlorothiazide: Potentiation of orthostatic hypotension w/ alcohol, barbiturates, narcotics or antidepressants; risk of lactic acidosis w. metformin; additive effect w/ other antihypertensive drugs; impaired absorption w/ cholestyramine & colestipol resins; intensified electrolyte depletion w/ corticosteroids, ACTH; decreased response w/ adrenaline; increased responsiveness w/ tubocuranine; reduced renal clearance w/ lithium; may raised serum uric acid w/ probenecid, sulfinpyrazone & allopurinol; increased bioavailability w/ atropine, MTX; enhanced toxic effects of salicylates; methyldopa; increased risk of hyperuricemia w/ cyclosporine; thiazide-induced hypokalemia or hypomagnesemia w/ digitalis glycosides; serum K disturbances w/ digitalis glycosides & antiarrhythmics; increased serum Ca levels; may interfere w/ parathyroid function; risk of hyponatremia w/ carbamazepine; increased risk of acute renal failure w/ iodine contrast media; hypokalemia w/ parenteral amphotericin B, corticosteroids, ACTH or stimulant laxative.
